Membership Application Forms
What you have to do to become a member
- Read this Application for Membership form including the Terms and Conditions of Membership on the back page.
- Fill out the Application for Membership and submit it with payment to:

- Make sure that your Nominator and Seconder are financial members of ASOS. Contact ASOS if you have any questions.
What ASOS will do
- Check that you are a registered medical practitioner.
- Confirm with the relevant ASOS State Chairman that your credentials as an Orthopaedic Surgeon are valid and acceptable under the Society's rules
(if we have not already been advised).
- Confirm with the ASOS Federal Treasurer that your Nominator and Seconder are current financial members of ASOS.
- Submit details, in accordance with the rules of the Society, after consultation with the ASOS Executive to the ASOS Federal Treasurer/ Membership Director (who can issue provisional membership).
- Approve or decline your membership application at the next general meeting of the Society.
- Notify you of the outcome.
Membership Fees
The membership fees* determined by the Society are:
- Entrance Fee $nil
- Emeritus Member $nil
- Ordinary Member $550
- Affiliate Member $66
- Registrar Member $33
- (Membership Fees are GST inclusive)
* Please note that membership fees are subject to change at any general meeting of the Society.
